Job Description
This position is part of a supportive housing case management team focused on homeless individuals and families. The CM-I will provide individual case management, including intake, assessment, goal setting, and referral, using Motivational Interviewing, Critical Time Intervention, and Stages of Change frameworks. Collaboration with other Services and Property Management Staff is key to achieving client goals.
Requirements
- BA in Social Services Field
- Two (2) years case management/client advocacy services with people who have been homeless and/or have a mental illness
- One (1) year experience utilizing a client tracking database system
- Proficiency with Microsoft software (Word, Excel, Power Point, Outlook)
- Sensitivity and appreciation of diverse tenant populations
- Fluency in English and Spanish
- Knowledge of the CHAMP and HMIS database systems
- Driving privileges
- Valid California Driver's License
