Case Manager I (ICMS)

This case management role within the Department of Health Services (DHS) provides Intensive Case Management Services (ICMS) to formerly homeless individuals and families with chronic medical conditions and co-occurring disorders. The CM-I (ICMS) will perform case management, providing individualized support and referral to relevant services and resources, with a focus on Motivational Interviewing and Stages of Change principles.

Requirements

• Work with a diverse caseload of tenants who have been marginalized and experience many barriers.
• Conduct individual assessments and provide appropriate level of services.
• Maintain daily and weekly contact with tenants.
• Collaborate with tenant to develop individual goal plans.
• Create and provide group supportive services.
• Maintain proper documentation in a computerized database system.
• Process housing applications and conduct interviews for permanent supportive housing.
