This case management role within the Department of Health Services (DHS) provides Intensive Case Management Services (ICMS) to formerly homeless individuals and families with chronic medical conditions and co-occurring disorders. The CM-I (ICMS) will perform case management, providing individualized support and referral to relevant services and resources, with a focus on Motivational Interviewing and Stages of Change principles.
