Job Description
We are seeking a Case Manager to develop, implement, and monitor Life Plans for individuals with developmental disabilities in our residential settings, ensuring services align with regulatory requirements, agency standards, and personal goals.
Requirements
- Bachelor's Degree in Human Services or related field
- Minimum of one (1) year of experience working with individuals with developmental disabilities
- Ability to read, write, speak, and understand English
- Strong interpersonal and communication skills with the ability to interact effectively with people supported, families, staff, and outside agencies
- Excellent organizational and time management skills with the ability to handle multiple priorities
