Case Manager

Under supervision of the Program Director, the Case Manager is responsible for developing, implementing, and monitoring Life Plans for the people we support in one of our residential settings. The role ensures that services align with regulatory requirements, agency standards, and the personal goals of each individual.

Requirements

• Bachelor's Degree in Human Services or related field required
• Minimum of one (1) year of experience working with individuals with developmental disabilities required
• Ability to read, write, speak, and understand English
• Strong interpersonal and communication skills with the ability to interact effectively with people supported, families, staff, and outside agencies
• Excellent organizational and time management skills with the ability to handle multiple priorities
• Proficiency in Microsoft Office and electronic record systems
• Ability to maintain confidentiality and exercise discretion in handling sensitive information
• Ability to work independently, as part of a team, and with flexibility in scheduling
• Ability to work flexible hours
