Provides social work services to individuals or groups, including screening, psychosocial assessment, discharge planning, and follow-up. Exercises professional judgment and makes recommendations for safe discharge planning. Documents activity according to department and program protocols.
Requirements
- Social Worker license issued by the state of practice
- Accredited Care Manager SW (ACM) certification or Certified Social Worker in Health Care (C-SWHC) certification within 1 year
- Master's degree in social work
- 5 years of experience in social work for a large, complex health care organization
- Excellent communication and human relation skills
- Intermediate computer skills, including word processing, presentation applications, and electronic mail
- Ability to integrate major social and health care issues into service provisions
- Demonstrated leadership skills in diplomacy, human relations, building rapport, collaboration, organization, prioritization, and promotion of staff development
