Carolinas Rehab Social Work Lead

Provides social work services to individuals or groups, including screening, psychosocial assessment, discharge planning, and follow-up. Exercises professional judgment and makes recommendations for safe discharge planning. Documents activity according to department and program protocols.

Requirements

• Social Worker license issued by the state of practice
• Accredited Care Manager SW (ACM) certification or Certified Social Worker in Health Care (C-SWHC) certification within 1 year
• Master's degree in social work
• 5 years of experience in social work for a large, complex health care organization
• Excellent communication and human relation skills
• Intermediate computer skills, including word processing, presentation applications, and electronic mail
• Ability to integrate major social and health care issues into service provisions
• Demonstrated leadership skills in diplomacy, human relations, building rapport, collaboration, organization, prioritization, and promotion of staff development
