Intensive Case Manger

Intensive Case Manager position at Affordable Living for the Aging (ALA) to provide case managing services to formerly homeless chronic individuals. The goal is to maintain the highest degree of independence, mental and physical health, and emotional well-being.

Requirements

• Bachelor's or Master's degree in social work or Mental Health related field
• At least one year of experience with the homeless population
• Experience working with clients with mental illness, chronic health issues, and substance use disorders
• Proficient knowledge of managed care organizations, Medicare, and Medi-Cal
• Excellent interpersonal skills and sensitivity to working with culturally diverse populations
