Primary Care Nurse Practitioner - Inland Empire | Bilingual Spanish/English

AllCare.ai is seeking experienced Primary Care Nurse Practitioners to provide on-site care to residents in Assisted Living and RCFE communities across the Inland Empire. The role involves conducting routine and urgent assessments, managing chronic disease, and building lasting relationships with residents and care teams.

Requirements

• Active California NP and RN licenses; active furnishing license
• Valid NPI, DEA, and BLS certification
• Bilingual fluency in Spanish and English required
• Experience in geriatrics, assisted living, RCFE, or facility-based primary care preferred (strongly preferred)
• EMR proficiency and commitment to same-day documentation
• Comfortable working independently in a mobile, field-based environment
