Primary Care Nurse Practitioner providing on-site care to residents in Assisted Living and RCFE communities across the Inland Empire, with a focus on geriatric care and building lasting relationships with residents and care teams.

Requirements

• Active California NP and RN licenses
• Valid NPI, DEA, and BLS certification
• Bilingual fluency in Spanish and English
• Experience in geriatrics, assisted living, RCFE, or facility-based primary care preferred
• EMR proficiency
• Comfortable working independently in a mobile, field-based environment

Benefits

• Annual compensation: $85,000 – $100,000
