Job Description
AllHealth Network is currently looking for a qualified experienced Case Manager to assist clients with complex issues and link clients with mental health resources. The MRT Case Manager will work with a medical clinician to divert mental health, substance use, and medical emergencies from law enforcement response. They will safety plan, determine needs, and navigate relevant mental health, substance use, and medical resources.
Requirements
- Bachelor's degree in a health/behavioral health related area of focus such as social work, psychology, sociology
- Minimum of 2 years of related experience with diverse populations in a physical and/or behavioral health setting
- Experience in case management, care coordination, and/or community health required
