Mental Health Partnerships (MHP) is seeking a full-time Case Manager to provide services to participants in the community with persistent mental health challenges. The role involves assessing needs, providing resources and referrals, and supporting participants towards self-sufficiency and positive outcomes. The position offers a hybrid work schedule, regular training, and opportunities for growth within the organization.

Requirements

• Collaborate with participants to create goals.
• Support participants with educational, employment, and social resources.
• Attend socialization activities.
• Document participant progress in electronic health records.
• Must have a valid driver's license.
• Must have access to a car.
• Must have automobile insurance coverage and current registration.
