Job Description
Mental Health Partnerships (MHP) is seeking a full-time Case Manager Trainee to provide services to participants living with persistent mental health challenges and co-occurring conditions. The role involves assessing needs, collaborating with participants, accessing resources, and supporting their goals for community living. MHP focuses on recovery and achieving participant goals with a commitment to excellence in workplace well-being.
Requirements
- Collaborate with participants to create self-directed goals.
- Support participants with educational, employment, and social resources.
- Attend social activities and document progress in records.
- Be a valid driver with automotive insurance.
