APC is searching for a CCS Youth Care Coordinator who can work in West Allis, WI and has the ability to transport clients by having a valid Wisconsin driver's license and vehicle and auto insurance. The ideal candidate will have an Associate’s degree or Bachelor's degree in human services and one-year related experience and/or training, or equivalent combination of education and experience.

- Ability to initiate contact with Comprehensive Community Services members and build rapport using a strength-based, trauma-informed, recovery-oriented, and culturally sensitive approach
- Ability to facilitate all aspects of Comprehensive Community Services service planning
- Ability to complete and submit all required or assigned documentation by due dates
- Ability to attend all supervision, meetings, training, in-services, and events as directed
- Ability to maintain members' records and ensure compliance with state and county expectations
- Ability to participate in quality improvement projects
- Ability to coordinate crisis services for members
