Out of Home Care Coordinator (Part-Time)

This position provides care coordination services to clients enrolled with White Mountain Apache Behavioral Health Services Tribal Regional Behavioral Health Authority. The Care Coordinator provides a link between the client, client support system, community, and other healthcare/social service professionals. The position will also act as an advocate for the client with other service providers as appropriate.

Requirements

• High school diploma or high school equivalency diploma
• Minimum of four years work related experience in mental health field
• Bachelor's degree preferred
• Ability to pass all pre-employment testing, including fingerprint clearance and background check
• One year experience working with Native Americans preferred
• Excellent written and oral communication skills
• Ability to speak the Apache language preferred
• Demonstrate critical thinking, problem-solving and organizational and time management skills
• Strong interpersonal skills
• MS Office proficiency
• Valid Arizona Driver's License and reliable transportation
