Job Description
We are searching for an experienced Director of Case Management to oversee inpatient care coordination, hospital transfers, and discharge planning.
Requirements
- Minimum of 3 years’ experience in a Case Management leadership position (Manager or Director level)
- 2+ years of related experience (Case Manager, Care Manager, Case Management, Care Management, Utilization Review, Utilization Management, Care Coordination)
- Current California RN License
