Job Description
Medical Facilities are looking for Physician - Nurse Anesthetist (CRNA) in Detroit, MI. Requirements include a clean file, 7+ shifts available per month, and a CV with all dates in MM/YY format. Availability and certifications are also required.
Requirements
- Provider must have a clean file and 7 plus shifts available per month.
- All candidates must have met the minimum shift availability listed on the job.
- CV must have all dates in MM/YY to MM/YY format
- Work History must be in Sequential order with most recent on top
- Any Gaps over 30 days must be explained on the CV
- EPA (Please use the attached EPA. Its also in Trio under your Name & then click on Help)
- All Licenses, Boards and Certs with expiration dates must be listed on CV
- Anyone with malpractice must provide a NPDB
- Anyone with legal issues must provide supporting documents
- Availability
- 1st possible date they would be available?
- How many shifts would they be able for per month?
- What month is their schedule completely open?
- Any shift preferences (Day, Night, Blocksetc)?
- Any dates in the next 3-6 months they cant work?
- Any Holidays in the next 3-6 months they would be open to work.?
