Home Health Social Worker MSW

PRN social worker opportunity to support patients on the north side of Indianapolis, must be a Master of Social Work (MSW) with at least 1 year of healthcare experience, preferably in hospice.

Requirements

• Assesses the psychosocial status of patients/clients and families/caregivers
• Carries out social evaluations and plans intervention based on findings
• Counsels patients/clients and families/caregivers as needed
• Maintains clinical records and prepares clinical and progress notes
• Educates patients/clients and families on advanced directives and practical needs
• Provides information and referral services for patients/clients and families/caregivers
• Maintains collaborative relationships with organization personnel
• Participates in quality assessment performance improvement teams and activities
