Job Description
The Anthony L Jordan Health Corporation (Jordan Health) is seeking a Behavioral Health Counselor to provide comprehensive diagnostic evaluations, treatment plans, and behavioral health consultations in an integrated primary care/behavioral health system.
Requirements
- Provide comprehensive diagnostic evaluations of mental health disorders based on DSM clinical criteria
- Assess and develop treatment plans to include medical and behavioral health concerns
- Help restore social conditions to enhance the patient/family's capacity to meet their personal and social needs
- Makes internal and external referrals to programs and agencies to foster/support resources to patients and families struggling with behavioral health disorders
- Provides Behavioral Health consultation to medical providers within the organization
