HEALTH HOME CARE MANAGER

The Health Home Care Manager will work in conjunction with a care team to identify and proactively manage the care needs of high-risk patients within the Federally Qualified Health Center practice setting. The role involves developing a plan of care, providing comprehensive care management, and facilitating care coordination with medical care and other services.

Requirements

• Develop a plan of care that promotes improved health care outcomes and quality of life
• Provide comprehensive care management including self-management support and connection to appropriate providers
• Facilitate care coordination with primary or specialty medical care and other care managers
• Provide culturally competent interventions based on patient assessment and identified cultural needs
• Review patient progress no less than quarterly
