Job Description
Jordan Health is seeking a Registered Nurse (RN) Triage to provide assessment, care coordination, advocacy, and coaching to patients within a Family Medicine setting. The RN Triage will collaborate with the director of nursing to deliver high-quality, patient-centered care, achieving health outcomes, financial objectives, and access goals. This role involves working with patients facing barriers to healthcare.
Requirements
- 1 year of nursing experience
- Current NYS RN License (BSN preferred)
- Current provider BLS/CPR Certification
- Infection Control Certification
