Foster Care Case Manager

The Foster Care Case Manager is responsible for ensuring the safety and services of children and families in foster care, including facilitating team meetings, developing individualized service plans, and documenting support related activities and services.

Requirements

• Bachelor's degree in social work, psychology, guidance and counseling, education, criminal justice, nursing, or related field human services field
• Regular face-to-face contact with referred child and foster family, at least bi-weekly
• Ability to transport assigned clients to and from home visits and other locations as necessary
