Case Manager

The Case Manager provides direct support and service coordination to individuals and/or families, using a client-centered approach to assess needs, develop service plans, and support clients in achieving their goals.

Requirements

• Conduct client assessments to identify needs, strengths, and goals.
• Develop, implement, and monitor individualized service plans.
• Meet with clients regularly to track progress and provide support.
• Maintain accurate and timely documentation of client interactions and services.
• Advocate for clients to access needed services and supports.
• Support clients in developing life skills and increasing self-sufficiency.
• Collaborate with team members and external partners to ensure continuity of care.
• Maintain confidentiality and comply with all applicable policies and requirements.
• Participate in team meetings and ongoing program activities.
• Perform other duties as assigned.
