Job Description
The Case Manager - Generalist is responsible for providing comprehensive care for individuals and families who are in crisis or homeless.
Requirements
- Communicate effectively both verbally and in writing with people of various socio-economic and cultural backgrounds
- Keep confidential information and only share client information on a need-to-know basis
- Use good judgment and organizational skills in evaluation situations, making referral decisions and following through
- Work harmoniously with applicants, recipients, general public and other employees
- Work in and promote a team environment
- Knowledge of Case management methods, principles and techniques
- Knowledge of Community and agency resources
- Knowledge of principles and methods of interviewing
- Skills in case documentation
- Skills in assessment case planning
- Skills in service planning, terminating cases and after care plans
