Job Description
The Association for Individual Development (AID) is a non-profit organization seeking a Case Manager Mental Health Professional New Start. The successful candidate will provide case management and coordination of behavioral health treatment to New Start clients, working with a team of behavioral health service providers and clients recently released from the Illinois Department of Corrections.
Requirements
- Attend to scheduled work hours and be flexible to meet client and program needs
- Provide effective face-to-face/telephone counseling service
- Complete and sign required documentation on a timely basis
- Provide Direct Service for clients, meeting monthly predetermined productivity standards
- Ensure that care provided integrates all facets of clients' lives
