Field Case Manager - LPN

We are seeking a Field Case Manager - LPN to provide exceptional home care services that enhance the quality of life for our patients and their families.

Requirements

• A graduate of an accredited school of nursing
• Must be a Licensed Practical Nurse and passed the state board
• Has experience in home health or public health nursing preferred but not required
• Knowledgeable about current nursing trends and legislative issues
• Excellent verbal and written communication skills
• Excellent administrative, organization and leadership skills
