Care Manager

Care Management is a service model focused on comprehensive caregiver needs management, including coordinating services to prevent hospitalizations and maintain health. Care Managers oversee assigned caseloads of children with perceived eligibility, utilizing problem-solving and care coordination to address behavioral health crises. This role involves assessment, plan development, engagement with clients, and advocating for patients in the healthcare system.

Requirements

• Completes initial and ongoing needs assessments (Child and Adolescent Needs and Strengths; CANS).
• Meets client contact requirements
• Meets Care Management documentation requirements
• Functions as an advocate for clients within the agency and external service providers.
• Promotes wellness and prevention by linking enrollees with resources and services
• Conducts care planning meetings/conferences and serves as an interdisciplinary team member
