Job Description
Peer Recovery Ambassador- NH position conducts community outreach to generate qualified treatment referrals, builds and maintains relationships with healthcare providers, and educates referral sources about Aware Recovery Care.
Requirements
- Conduct community outreach to generate qualified treatment referrals.
- Build and maintain relationships with healthcare providers, recovery communities, sober homes, clinicians, and community partners.
- Educate referral sources about Aware Recovery Care, available services, treatment programs and recovery services offered by the company.
- Develop and maintain a portfolio of relationships with referral sources within the assigned region;
- Maintain accurate documentation of outreach activity and referral development efforts for assigned region.
- Communicate professionally and ethically with referral partners, clients, families and the general public.
- Meet established outreach and referral development goals as assigned by Aware and supervisor.
- Represent Aware Recovery Care in a professional and recovery focused manner.
- Coordinate with the cross functional departments and teams to ensure smooth handoffs from initial community contact to enrollment in care.
- Perform other related duties as assigned.
