Job Description
The Case Manager will provide stabilization services to families in need of family preservation services within the Kent, OH office and client homes. This role involves caseload management, crisis intervention, assessment, and collaboration with a variety of community resources. The position requires travel to clients' homes and strong community engagement.
Requirements
- Maintain caseload of up to 15 families living in Portage County.
- Meet minimum productivity standard as established by program need and clinical leads.
- Travel to clients’ homes and provide services in the community setting.
- Verify Ohio Driver’s License and meet agency requirements for driving insurance.
- Adhere to agency policies and procedures, COA regulations, and educational/certification/registry requirements.
