Chemical Dependency Targeted Case Manager

MedMark Treatment Centers is looking for a Targeted Case Manager to enhance the overall treatment process by assisting counselors in meeting the overall treatment needs of the patients.

Requirements

• Provide guidance and assistance in accessing multiple levels of care
• Communicate with patient prior to start of treatment or within the first week of treatment
• Develop a treatment plan/services plan with the patient that identifies key needs promoting successful recovery
• Facilitate individual and group Case Management as assigned
• Identify, build and cultivate relationships within the referral community
• Ensure effective cross-communication and collaboration with the Intake team
