Job Description
Bayshore HealthCare is seeking a Manager of Clinical Practice to lead our Bundled Care Program, supporting clients and caregivers through high-quality, community-based care. The role includes 1 to 2 in-office days per week and follows a rotating schedule between 8:00 a.m. and 7:00 p.m., Monday to Friday.
Requirements
- Minimum 5 years in healthcare, preferably in home care and hospital settings.
- Registered Practical Nurse (Ontario), graduate of an accredited program, current CPR certification.
- Strong understanding of community nursing, service delivery, discharge planning, and Ontario's health system.
- Proven ability to mentor and lead teams with compassion and consistency.
- Familiarity with RAI tools is an asset.
- Valid driver's license and reliable vehicle.
