Job Description
Bayshore HealthCare is seeking a Manager of Clinical Practice to lead our Bundled Care Program. This is a one-year contract with a hybrid work model requiring onsite presence two days per week at our Kingston location.
Requirements
- Minimum 5 years in healthcare, preferably in home care and hospital settings.
- Registered Practical Nurse (Ontario), graduate of an accredited program, current CPR certification.
- Strong understanding of community nursing, service delivery, discharge planning, and Ontario's health system.
- Proven ability to mentor and lead teams with compassion and consistency.
- Familiarity with RAI tools is an asset.
- Valid driver's license and reliable vehicle.
