Behavioral Health Case Manager

This Behavioral Health Case Manager will be responsible for risk assessments, patient education, tracking patient needs and referrals, and facilitating continuity of care. The role involves assisting patients with Medicaid, WIC, and other entitlement programs, counseling patients on medication adherence and mental health, and completing service documentation. The case manager will be on-site and report to The Director of Behavioral Health services.

Requirements

• Previous experience in a licensed ambulatory health care facility and a minimum of two years of direct experience preferable in primary care.
• Possess culturally competent skills/experience working with diverse populations.
• Ability and desire to provide service to patients consistent with Florida State Health Department and the Bureau of Primary Health Care Program guidelines.
• Bachelor’s degree in a human service field.
• 2 or more years of case management and counseling experience.
