Job Description
Denova Collaborative Health LLC is seeking a Case Manager to assist in patient care for behavioral health services. The role focuses on coordinating care to enhance education, self-management support, and timely healthcare delivery. The Case Manager will serve as an advocate for patient healthcare needs and facilitate access to necessary services within the healthcare system. This is a pay-for-performance (P4P) position.
Requirements
- Bachelor’s Degree in Counseling, Psychology, Social Work, or a related field preferred
- Associate’s Degree with 2-3 years of experience in a related field
- High School Diploma or equivalent with 3 years of related experience
- Qualified as a Behavioral Health Technician
- CPR/First Aid certification and a valid Arizona fingerprint clearance
- Excellent communication and time management skills
- Exceptional problem-solving skills and the ability to work within an interdisciplinary team
- Independent work ability
