Job Description
Boston Health Care for the Homeless Program is seeking a Community Access Outreach Specialist/Data Coordinator to provide medical case management and community access services to patients in an outreach setting. The role involves driving a van to mobile outreach sites, providing education on overdose prevention, and distributing Narcan rescue kits. The successful candidate will have experience in community access services, medical case management, and data management, with a commitment to equitable and culturally appropriate care for diverse populations.
Requirements
- Current and valid Massachusetts driver’s license
- At least two years of experience providing community access services or medical case management
- Experience in data management and competence in Excel
- Phlebotomy skills
- Understanding of multicultural issues, substance use, mental health, and complex health problems of people experiencing homelessness
