Community Access Team Specialist

Boston Health Care for the Homeless Program seeks a Community Access Team Specialist to provide street-based outreach services, care coordination, and case management to individuals experiencing homelessness in the South End. The role requires a commitment to harm reduction strategies and trauma-informed approaches, with a focus on delivering equitable and culturally appropriate care to diverse populations.

Requirements

• Collaborate with CAT Specialists, nurses, and medical providers to engage with people who use drugs in a low-threshold, street-based setting.
• Serve as a liaison to connect patients with longitudinal case management programs and assist in transitions.
• Provide culturally responsive, trauma-informed services, including sexual health promotion, substance use risk reduction, medical care coordination, social services coordination, and medication adherence support.
