Case Manager, LINK

We are seeking a Case Manager to provide outreach and case management services to people experiencing homelessness in Queens, NY. The Case Manager will assist clients in achieving housing and recovery goals, connect them to psychiatric and medical services, and support them in obtaining permanent housing.

Requirements

• Bachelor’s Degree or equivalent work experience
• Two years of experience working with homeless adults
• Experience with harm reduction and motivational interviewing techniques strongly preferred
• Valid NY, NJ, CT, or PA driver’s license with a good driving record
• Excellent computer skills, including proficiency in Microsoft Word and Excel
