Case Manager, Outreach

We are seeking a Case Manager, Outreach to provide case management services to people experiencing homelessness, with a focus on transitioning them to permanent housing. The role involves providing direct services, conducting assessments, connecting clients to psychiatric and medical services, and assisting with housing applications.

Requirements

• Bachelor’s Degree or equivalent work experience
• Two years of experience working with homeless adults and substance abuse issues
• Experience with harm reduction and motivational interviewing techniques
• Valid NY, NJ, CT, or PA driver’s license with a good driving record
• Excellent computer skills, including proficiency in Microsoft Word and Excel
• Excellent writing and communication skills
• Bilingual Spanish/English is preferred
