The Case Manager, Woodhull provides outreach and case management services to people experiencing homelessness, assisting them in transitioning to permanent housing. This position requires completion of a 'Master Agreement Health Clearance' and attestation of compliance with New York State Department of Health requirements. The Case Manager will conduct assessments, benefits assistance, and other services necessary to support clients in achieving housing and recovery goals.
