Job Description
We are seeking a Case Manager, Transitional Housing to provide case management services to individuals experiencing homelessness, helping them transition to permanent housing. The role involves conducting assessments, benefits assistance, and connecting clients to psychiatric and medical services.
Requirements
- Bachelor’s Degree or equivalent work experience
- Two years of experience working with homeless adults and substance abuse issues
- Experience with harm reduction and motivational interviewing techniques
- Valid NY, NJ, CT, or PA driver’s license with a good driving record
- Excellent computer skills, including proficiency in Microsoft Word and Excel
- Excellent writing and communication skills
- Bilingual Spanish/English is preferred
