Job Description
We are seeking a Case Manager, Transitional Housing to provide case management services to people experiencing homelessness in order to assist them in transitioning to permanent housing. The position has a 35-hour work week and involves conducting assessments, benefits assistance, and connecting clients to psychiatric and medical services.
Requirements
- Bachelor’s Degree or equivalent work experience
- Two years of experience working with homeless adults and substance abuse issues preferred
- Valid NY, NJ, CT, or PA driver’s license with a good driving record, as defined by Breaking Ground is preferred
