Job Description
The Case Manager is responsible for supporting clients within the case management program, communicating with clients and their teams, and developing and revising reintegration plans.
Requirements
- Bachelors degree in Social Work, Psychology, or a related field
- 1-2 years of experience in case management or a related field
- Ability to work independently and as part of a team
- Strong communication and interpersonal skills
- Ability to prioritize tasks and manage multiple projects
