Case Manager

Join Saint John's Program for Real Change as a Case Manager and become part of a community dedicated to making a real difference in the lives of those we serve. As a Case Manager, you'll work alongside clients to help them identify and meet personal goals, while adhering to the milestones and expectations of the program at large. You'll be responsible for conducting thorough assessments, developing individualized service plans, and providing case management to clients and their families.

Requirements

• Social Work or Psychology Bachelor's degree plus 1 year of experience; OR AA degree plus 2 years of experience working with individuals or families with complex needs
• Works independently, flexibly, and as part of a team
• Works effectively with outside agencies in providing client services
