Job Description
We are looking for a Medical Case Manager I to coordinate and deliver comprehensive case management services to clients impacted by HIV, Hepatitis C, STI's, and other infectious diseases. You will become part of our professional team that drives home our Company's Mission and Values.
Requirements
- Bachelor's degree in social work, Nursing, Public Health, or related field
- One year of HIV prevention/intervention experience
- Must be able to operate a motor vehicle and have valid insurance and driver's license
- Must be able to pass a Level I and Level II Background check as required
