Job Description
The Case Manager (Lifespan) will provide case management and wrap-around services for clients accessing the Mobile Mental Health & Addictions Clinic. The Case Manager will assess, plan, develop, coordinate, connect and support services to program clients.
Requirements
- BSW preferred, or demonstrated equivalent
- Knowledge of mental health/illness and mental health difficulties and treatment modalities
- Assessment, advocacy, and evaluation skills
- Ability to work collaboratively with clients, team colleagues, and community stakeholders
- Valid Driver's License
- Criminal Records Check
- Certification in CPR and First Aid
