Job Description
The Canadian Mental Health Association – Peel Dufferin Branch is seeking an Intensive Case Manager to provide case management supports to individuals experiencing serious mental illness and addictions concerns. The Intensive Case Manager will work in collaboration with internal and external programs and agencies to address client’s unmet needs using a stepped care approach.
Requirements
- Post-secondary degree or diploma in relevant discipline (e.g. BSW, MSW, OT, RN)
- Being a registered and/or regulated health professional is an asset
- Expertise in navigating community mental health and/or community-based human services
- Minimum 2 years of experience with a focus on assessment, brief interventions, and short-term supports
