Intensive Case Manager

As an Intensive Case Manager, you will provide housing support and services to persons who are chronically or episodically homeless in Peel Region, Ontario, Canada. You will work with a multidisciplinary team to build a collaborative care plan and consult with other disciplines as needed. You will also participate in regular staff meetings and care planning to provide case management expertise for client care within a multi-disciplinary team.

Requirements

• Post-secondary degree or diploma in a human services field.
• Minimum 2 years' experience working with vulnerable populations.
• Experience with case management in a community setting required.
• Demonstrated knowledge of serious mental illness, concurrent disorders and/or dual diagnosis required.
• Knowledge of Critical Time Intervention (CTI) is an asset.
• Knowledge of Housing First Principals is an asset.
• Comprehensive knowledge of Recovery Principles.
• Valid Canadian driver's license and access to an insured automobile in good repair.
• A satisfactory Vulnerable Sector Screening (Police Check) report.
