Requirements
- Certification with the College of Family Physicians of Canada (CFPC)
- Eligibility for licensure to practice as a Family Physician in the Province of Ontario
- Canadian Medical Protective Association (CMPA) coverage (or eligibility to obtain)
- Commitment to working within and optimizing an interdisciplinary team
