Job Description
Clinical Pharmacist role at a patient-facing practice, working with a multi-disciplinary team to manage chronic diseases, provide clinical guidance, and optimize patient care.
Requirements
- Clinical guidance on medicines optimization and quality improvement
- Review and implement practice repeat prescribing policies
- Conduct clinical medication reviews and polypharmacy management
- Provide clinical medicines advice and address public and social care needs
- Support efforts to reduce medicine-related hospital admissions and readmissions
