Job Description
We are seeking a Case Manager for our Care Management for Adults & JACC program. The Case Manager is responsible for developing and managing a service plan for clients, providing direct counseling services, and establishing continuity of care. Requirements
- Bachelor’s Degree in a related field
- 1 year of related experience
- Valid NJ driver's license
- Bilingual preferred
