The Case Manager Lead provides case management and clinical services, team leadership, and operational support to the Crisis Services team. This position is responsible for ensuring high quality crisis intervention and supportive services are provided to individuals and families on a 24/7 basis.
Requirements
- Provides crisis intervention, stabilization, and case management services to clients
- Assists clients with skill-building and supports such as activities of daily living, psychosocial support, transportation, and referrals to other community resources
- Coordinates treatment with and initiates referrals to other providers as appropriate
- Completes progress notes and other required documentation within the required timeframe
- Provides ongoing training, guidance, and field-based coaching to staff on the assigned Crisis Services team
- Fosters teamwork, cooperation, and respectful communication between team members
- Ensures that team members adhere to established procedures around clinical documentation
- Promotes a culture of privacy and security of Protected Health Information within a HIPAA covered organization
