Job Description
Catholic Home Health Services is seeking a Registered Nurse to provide direct and indirect patient care in a home setting. This position requires adhering to current regulations and policies, offering quality care to patients, and contributing to a collaborative team environment. The RN will be responsible for assessing patient needs, developing care plans, and coordinating services.
Requirements
- Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients.
- Assists patients in the recovery or maintenance of life functions; prevent illness and relapse of illness.
- Develop, initiate, and revise the Plan of Care (POC) as necessary.
- Coordinate services and informs the physician and other personnel of changes in the patient’s condition and needs.
